Under  art. 13 (1) and (2) of the Regulation (EU) 2016/679 of the European Parliament and of the Council  of 27 April 2016 (the GDPR)  we prepared a set of required information in this regard:

3. How did we obtain your personal data?

We obtained them from you when you were purchasing services in our company and also later during telephone conversations and e-mail correspondence.

4. Is the provision of personal data voluntary?

The provision of personal data for concluding a contract and the provision of services is voluntary, however, the consequence of not providing personal data will be the inability to provide the chosen service.

5. Why do we proces your personal data?

Your personal data are processed for the following purposes:

conclusion and execution of a contract, marketing and promotion of MTD Skuratowicz services, issuing invoices and accepting payments, debt collection and defense of any claims,  handling claims and declarations, for tax, accounting and archiving purposes.

6. How long will your personal data be processed?

Your personal data will be processed for the period necessary to achieve the indicated goals, in accordance with the criteria set out in the provisions of law.

In the case of processing for marketing purposes based on your consent (Article 6 (1) (a) of the GDRP), until you withdraw your consent.

7. How do we share your personal data with?

Your personal data may be disclosed to the following categories of recipients:

• authorized entities based on legal regulations being in force;

• the entities processing data on our behalf, i.e. the entities processing data as sub-contractors based on the contracts concluded with us and acting solely in compliance with our instructions.

8. Do we transfer your data to third countries outside the European Economic Area?

Your personal data may be transferred outside the EEA if it is necessary to provide a given service for you.

9. What rights do you have?

the right to request access to your personal data (Art. 15 of the GDPR), their rectification (Art. 16 of the GDPR), erasure (Art. 17 of the GDPR),  restriction of your data processing (Art. 18 of the GDPR) or the right to object to your data processing (Art. 21 of the GDPR) and also the right to data portability (Art. 20 of the GDPR);

the right to withdraw your consent at any time - if the processing is carried out on that basis, without prejudice to the lawfulness of processing carried out on the basis of the consent before its withdrawal pursuant to Art. 6 (1) (a) or Art. 9 (2) of the GDPR;

the right to lodge a complaint with the supervisory authority - President of the Office for Personal Data Protection – if you believe that the manner of processing your personal data breaches the GDPR or other provisions related to personal data protection;

10.  Do we profile your personal data?

Your personal data will not be subjected to automated decision making, including profiling, referred to in Art. 22 (1) and (4) of the GDPR.
